RECORDING IDENTIFICATION; The name choices for horse, foaling date,sex, color, markings and parent information. Provide recent photos of both sides, front and back of horse.
OWNER INFORMATION; The recorded owner of the Dam at the time of breeding is responsible for the completion of the section and will be recorded as the original owner of the foal.
BREEDER CERTIFICATE: The recorded owner of the Dam at the time of breeding is responsible for the completion of this section. Date of breeding and how the mare was bred must be indicated.
SERVICE CERTIFICATE: The recorded owner of the Stallion is responsible for the completion of this section. Date of breeding and how the Mare was bred.
TRANSFER REPORT: The recorded owner if the Dam at the time of foaling is responsible for the completion of the section if the foal is to be recorded in a name other than the owner at the time of foaling. Signatures of both the seller and buyer are required.
FEES: (U.S.D.) must accompany application. Registration fee is $10.00 for members, $20.00 for non members. Transfer fee is $10.00, if there is a transfer at the new time of registration, the total registration fee is $20.00, for ASHA members. Fees are unchanged regardless of date of transactions or foaling. Changes or replacement certificates will incur a $10.00 fee. Rush delivery is available for an additional $20.00, certificate will be processed within the week of receipt.
Applicants signing this form certify that all information submitted is true and correct to their personal knowledge and agree that the registry has the privilege to correct and/or cancel the registration certificate for the cause under its rules and regulations. The registry is not responsible for the accuracy of statements made on the application form. Any horse registered or transferred through misrepresentation or fraud, to together with its descendents shall be deleted from the Registry.
